Student Appointment Request for Proctored Test

Please fill out the form below and click the SUBMIT button. NOTE! Please send your request AT LEAST 24 HOURS PRIOR to your requested appointment time. When entering the time length for your test, give the total time allowed for the test by your instructor, and add 30 minutes. We must be able to schedule you to finish by 4:30 p.m. M-F. Be sure to include the time as well as the date. We cannot confirm appointments with no time specified. If there is no time limit, give a generous estimate of the time you expect the exam will require. Note: This form is for use by Vincennes University Distance Education students or students enrolled at other colleges or universities who require proctor services for testing.
