Print out your permission slip and mail it to the address at the bottom of the page.  Remember...if the trip you are signing up for is during the school day, you must have a school official sign the bottom of the form.  Parents are always welcome and encouraged to attend trips with Scholars.

REMEMBER:  Please write the event that you are signing up for at the top of the page!
