Procedures for Requesting Academic Accomodations

The Office of Disability Services assists all eligible students with disabilities by providing reasonable accommodations.

In the college setting, it is the student’s responsibility to initiate the request for any individual assistance.

  1. Students with a disability who need accommodations should identify themselves and request assistance from the Office of Disability Services during the first week of class.
  2. Appropriate documentation supporting the request for accommodation must be submitted for evaluation purposes.  See the guidelines for documentation for the relevant disability.
  3. The Office of Disability Services is available to meet with students on an individual basis to discuss the accommodation process.  Students who are eligible will receive copies of the accommodation form, and be advised of the procedures for implementation.
  4. To initiate accommodations, it is the student’s responsibility: 1) to provide the instructor with the accommodation form, and 2) schedule a meeting if necessary, to discuss implementation of the accommodation. Students are strongly encouraged to submit accommodation forms to their instructors at the beginning of the semester in order to receive the optimum benefit.
  5. Due to the volume of information received, documentation may not be reviewed until an accommodation request is made. 
  6. Students receiving accommodations must renew their request for services each semester with Disability Services.
