FLEXIBLE SPENDING (SECTION 125)

A Flexible Benefits Plan through Chard Snyder will:
- Boost your take-home pay
- Lower your medical costs
- Cut your income taxes
- Reduce your day care costs
It works like an expense account. You automatically set aside part of your salary
(before taxes) to pay for qualified medical, dental and vision expenses or dependent
care. You save money because you don’t pay taxes on the money you set aside. And you
can use it to pay for day care, medical and prescription drug costs that aren’t covered
by insurance, as well as over-the-counter medicines and drugs you buy every day. It’s
a great perk from your employer and it’ll save you money.
Effective January 1, 2020, the IRS maximum elections are currently set to be $2,750 for Medical FSAs and $5,000 for Dependent Care FSAs.

WELLNESS PROGRAM
TOBACCO CESSATION PROGRAM
One of the greatest strides we can take as an employer towards overall health and
wellness is to promote and assist with tobacco product cessation. More than 40,000
scientific studies have linked smoking and the usage of other types of tobacco products
with adverse health effects. Cigarette smoking is the leading cause of preventable
death in the United States and produces substantial health-related economic costs
to society.
All employees covered under a Vincennes University health care plan must certify annually
whether you and/or your spouse (if covered under VU’s plan) use tobacco products or
are tobacco-free by completing the online Tobacco Affidavit under the Employee tab
of the VU-x web portal. Tobacco includes any form of tobacco products that are smoked
(e.g. cigarettes, cigars, pipes), applied to the gums (dipping and chewing tobacco),
and/or inhaled as snuff. Tobacco users on the Vincennes University health care plan
will pay a $35 biweekly surcharge.
Plan members that currently use tobacco products will be given the opportunity to
receive tobacco cessation assistance. The VU group health plan will cover 100 percent of the cost of tobacco
cessation prescription drugs. Coverage is limited to a 180-day supply per covered
individual per year. Covered prescription drugs include Chantix, Zyban (bupropion),
and Nicotrol.
If you are not tobacco free, we hope you will take advantage of this opportunity to

CANCER CARE
The University offers Cancer Care Insurance through Aflac. This benefit provides a
lump sum benefit if you are diagnosed with one of the covered illnesses.
Eligibility
Full-Time employees can obtain coverage for yourself, spouse and children. This election
can only be added or removed during an open enrollment period.
Cost
If you elect Cancer Care coverage the premiums will be paid through a payroll deduction.
The employee is responsible for 100% off the premiums for this benefit.
Forms
DENTAL (HRI)

The University offers Dental Insurance through HRI.
Eligibility
Full-Time employees can obtain coverage for yourself, spouse and children. This election
can only be added or removed during an open enrollment period.
Cost
If you elect Dental coverage the premiums will be paid through a payroll deduction.
The employee is responsible for 100% off the premiums for this benefit.

The University offers Vision Insurance through VSP.
Eligibility
Full-Time employees can obtain coverage for yourself, spouse and children. This election
can only be added or removed during an open enrollment period.
Cost
If you elect Vision coverage the premiums will be paid through a payroll deduction.
The employee is responsible for 100% off the premiums for this benefit.

The University offers Whole Life Insurance through Boston Mutual.
Eligibility
Full-Time employees can obtain coverage for yourself, spouse and children. This election
can only be added or removed during an open enrollment period.
Cost
If you elect Whole Life insurance the premiums will be paid through a payroll deduction.
The employee is responsible for 100% off the premiums for this benefit.
